CHECKLIST OF REQUIREMENTS
For the Bidding of Infrastructure Project
Revised September 2014

Bidders shall submit their duly authorized representatives in two (2) separate sealed bid envelopes, pursuant to Section 25.1 of the Revised IRR of 9184, otherwise known as the “Government Procurement Act.”

The two (2) separate sealed envelopes shall contain the following:

I. FIRST BID ENVELOPE shall contain the:  A. Eligibility Requirements  and B. Technical Component of the Bid (Section 25.2)

A. Eligibility Requirements (Section 23.1)

1. PhilGEPS Certificate of Registration and Platinum membership in accordance with Section 8.5.2 of this IRR, except for foreign bidders participating in the procurement by a Philippine Foreign Service Office or Post, which shall submit their eligibility documents under Section 23.1 of this IRR: Provided, That the winning bidder shall register with the PhilGEPS in accordance with Section 37.1.4 of this IRR;
2. PCAB License and Registration
3. Statement of all Ongoing Government and Private Contracts.
4. Statement of Single Largest Completed Contract (SLCC)
5. NFCC Computation, (at least equal to ABC)
6. JVA, if applicable

B. Technical Component of the Bid: (Section 25.2)

1. Bid Security in the prescribed form, amount and validity period or Bid Securing Declaration.
Project  Requirements, which shall include the following:
a) Duly signed Organizational Chart for the contract to be bid
b) Duly signed List of Contractor’s personnel to be assigned to the contract to be bid, with their complete qualification and experience data
c) Duly signed List of Contractor’s major equipment units, which are owned, leased and/or under purchase agreements, supported by proof of ownership or certification of availability of equipment from the equipment lessor /vendor for the duration of the project , as the case may be
2.     Omnibus Sworn Statement (Section 25.3) by the bidder or its duly authorized representative in the form prescribed by the GPPB stating that:
a) The signatory is duly authorized
b) It is not “blacklisted” or barred from bidding
c) Each  of the documents submitted is an authentic copy of the original, complete, and all statements and information provided therein are true and correct
d) It is authorizing the Mayor or his duly authorized representative /s to verify all the documents submitted
e) It is not related to the Mayor, members of the BAC, the TWG, and the BAC Secretariat, the head of the PMO or the end –user unit, and the project consultants by consanguinity of affinity up to the third civil degree
f) It complies with the existing labor laws and standards
g) It complies with the responsibilities of a prospective or eligible bidder
h) It is aware of and has undertaken the responsibilities as a bidder (a) Carefully examine all of the bidding documents (b) Acknowledge all conditions, local or otherwise, affecting the implementation of the Contract (c) Made an estimate of the facilities available and needed for the contract to the bid, if any (d) Inquire or secure Supplemental /Bid Bulletin (s) issued for the project
i) It did not give or pay, directly or indirectly, any commission, amount, fee or any form of consideration pecuniary or otherwise to any person or official, personnel pr representative of the government in relation to a procurement project or activity
3.     For the foreign bidders claiming eligibility by reason of their country’s extension of reciprocal rights to Filipinos, a     certification from the relevant government office of their country stating that Filipinos are allowed to participate in their       government  procurement activities for the same item or product.

II. SECOND BID ENVELOPE shall contain the Financial Component of the bid (Section 25.3)

1. Financial Bid Form, which includes bid prices and the Bill of Quantities
2. Any other document related to the financial component of the bid, any

Note:      Preliminary examination of bids will be conducted using the non discretionary “pass /fail criterion as provided for in Section 30, Rule XI of the Revised IRR of R.A 9184
Bid that fail to include any requirement or are incomplete or patently insufficient shall be considered as “Failed”
*Please submit required documents in one (1) original copy and two (2) certified true copies each.
*Please attach Official Receipts paid for the Bidding Documents and Bidder’s Permit in the 1st Bid Envelope

1. Bid Security in the prescribed form, amount and validity period. (Note: Manager’s Check should be issued by the local banks) Or Bid Securing Declaration.
2. Technical Specification of the items to be bid stating “COMPLY” if the items being offered  complies with the Technical Specifications required by the Municipality of Pototan in the Bidder’s Tender
a) Certificate stating the Production/Delivery schedule in accordance with the ITB, IF any
b) Certificate of After-sales service/parts as instructed in the ITB, if any
c) List of Manpower and supporting documents (ex. Copy of PRC License, Certificate of Training) as instructed in the ITB, if any
